Hospital care spending rose 10.4% to $1.5 trillion in 2023, marking the fastest growth since 1990. Hospital discharges increased by 1.6%, with Medicare beneficiaries driving higher outpatient service utilization.
Spending on physician and clinical services also climbed by 7.4% to $978 billion, with private insurance and Medicare patients contributing to this rise.
Enrollment in private health insurance grew by 1.6%, adding 3.3 million Americans.
Private insurance spending surged by 11.5%, reaching $1.5 trillion and accounting for 30% of national healthcare expenditures.
Medicare spending grew 8.1% to $1 trillion, representing 21% of total healthcare spending. Medicaid spending, however, grew more slowly at 7.9%, reflecting the end of the COVID-19 public health emergency.
The report also revealed that 92.5% of Americans had health insurance in 2023, up from 92.0% in 2022, underscoring the broader coverage expansion in recent years.

One of Siri&#8217;s criticisms of vaccines is that they&#8217;ve not not undergone randomized, double-blind clinical trials; he says they should be removed from the market until such trials are conducted. This approach would involve denying millions of American children access to polio shots, which public health experts consider unethical due to the known risks of polio. 
"You're substituting a theoretical risk for a real risk," <a href="https://www.nytimes.com/2024/12/13/health/aaron-siri-rfk-jr-vaccines.html">said</a> Dr. Paul A. Offit, a vaccine expert at the Children's Hospital of Philadelphia, according to the Times. 
Polio vaccination has been part of the routine childhood immunization schedule in the United States for decades.
Though Kennedy has stated that he does not intend to restrict access to any vaccines, his ongoing collaboration with Siri raises concerns that vaccine policy could face intensified scrutiny. 
<h2 class="wp-block-heading">What Say You, Donald Trump?</h2>
In an <a href="https://time.com/7200212/person-of-the-year-2024-donald-trump/">interview</a> published Thursday by Time magazine, Trump said he's going to direct Kennedy to study the rising rates of autism diagnoses and that he'd consider getting rid of some vaccines for children, falsely suggesting autism may be caused by vaccines.
"We're going to have a big discussion. The autism rate is at a level that nobody ever believed possible. If you look at things that are happening, there's something causing it," Trump said.
<h2 class="wp-block-heading">Bad News For Pharma Companies</h2>
Pharmaceutical companies that produce either standalone polio vaccines or in combination with other immunizations &#8211; GlaxoSmithKline (NYSE:<a class="ticker" href="https://www.benzinga.com/stock/GSK#NYSE">GSK</a>), Sanofi (NASDAQ:<a class="ticker" href="https://www.benzinga.com/stock/SNY#NASDAQ">SNY</a>) and the privately owned Bilthoven Biologicals &#8211; are not likely to view Kennedy and Siri's anti-vaccine push in a positive light.
Pfizer Inc. The Latest On Bird Flu: A recent research study from the University of Glasgow found the virus is possibly spreading to horses in Mongolia and suggests the horses could spread the flu to humans. Professor Pablo Murcia, who led the research, <a href="https://news.sky.com/story/virus-most-likely-to-cause-next-pandemic-could-be-passed-to-humans-from-horses-study-finds-13271625" target="_blank" rel="noopener">told</a> SkyNews that knowledge of wildlife infections will help scientists monitor the virus.
Two cats in Los Angeles died from consuming raw milk that contained the virus, KTLA <a href="https://ktla.com/news/local-news/2-cats-die-from-possible-h5-bird-flu-in-los-angeles-county/">reports</a>. Cats can be exposed to the flu by eating infected birds or consuming unpasteurized milk from infected cattle. The Los Angeles County Department of Public Health noted that cats have reportedly been infected with the virus after drinking unpasteurized milk elsewhere.
Officials at the Wildlife World Zoo in Litchfield Park, Arizona, announced that five animals at the zoo died of avian flu, <a href="https://www.abc15.com/news/state/bird-flu-virus-hits-wildlife-world-zoo-park-implementing-safety-measures">according</a> to ABC15. The perished animals include a cheetah, a mountain lion, a swamphen, an Andean goose, and a kookaburra. A white tiger has also tested positive but is expected to make a full recovery.
In late November, a human in Canada who had been infected by the flu <a href="https://www.benzinga.com/general/health-care/24/11/42087908/mutated-bird-flu-case-in-california-puts-humans-at-increased-risk-so-which-companies-are-developing-vaccines">showed indications of mutation</a>. This is a sign the flu could spread more easily.

Bird Flu Spreads To Cats, Possibly Horses As Officials Signal No Plans To Authorize Vaccination

During 2000–2020, the U.K.'s National Health Service (NHS) faced significant trade-offs in prioritizing new drug treatments, as resources allocated to these medications often came at the expense of broader population health gains.
Lancet research revealed that while newly approved drugs directly benefit certain patients, the overall impact on population health was negative due to the high costs and resulting resource reallocation.
A retrospective analysis reviewed technology appraisals conducted by the National Institute for Health and Care Excellence (NICE) on 332 drugs.
Of these, 276 (83%) received positive recommendations, and most were appraised within five years of regulatory approval.
The study ultimately included 183 drugs after excluding ineligible cases. While these drugs generated an estimated 3.75 million quality-adjusted life years (QALYs) across 19.82 million patients, their cost to the NHS—totaling $94.93 billion (75.1 billion pounds)—led to an estimated net loss of 1.25 million QALYs.
For example, oncology drugs demonstrated a median cost-effectiveness ratio (ICER) of 30,000 pounds per QALY gained, significantly higher than the 6,478 pounds for anti-infective drugs.
Additionally, the ICER for new drug recommendations rose by 32% from 2000 to 2020, reflecting increased financial pressures on the NHS.
Despite potential long-term benefits from price reductions after patent expiration, these effects were limited. Only 19% of the studied drugs had generic or biosimilar alternatives by 2020.
For new drugs to contribute positively to population health, their initial costs would have required a median reduction of 42%.
The ongoing debate around NICE's cost-effectiveness threshold remains central to policy discussions.
A 2024 agreement retained the current threshold until 2029, but experts argue for alignment with the Department of Health and Social Care's estimate of health-opportunity costs.
Such an adjustment could ensure the NHS achieves better value for its investments in new drugs without compromising care quality in other areas.
Balancing innovation with equitable health outcomes requires nuanced policy solutions. The study <a target="_blank" href="https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(24)02352-3/fulltext" rel="noreferrer noopener">suggests</a> a lower cost-effectiveness threshold, closer to 15,000 pounds per QALY, could better align spending with health gains.
However, opposition from the pharmaceutical industry underscores the challenges of reforming pricing policies while fostering innovation.
This approach highlights how benefiting one group of patients may require deprioritizing others as existing NHS services and treatments are displaced to fund new drugs.
For instance, in 2010, NICE approved Roche Holding AG's (OTC:<a class="ticker" href="https://www.benzinga.com/stock/RHHBY#OTC">RHHBY</a>) Herceptin (trastuzumab) for metastatic gastric cancer, deeming an ICER of 43,206 pounds per QALY gained as cost-effective.
Framing this as 2.88 QALYs forgone for every QALY gained clarifies the value placed on patients receiving the treatment versus those who might lose out on care.
At a population level, using trastuzumab in this context resulted in an estimated 4,000 QALYs lost between 2010 and 2020.
However, adopting this perspective could pose challenges for policymakers due to the established 20,000–30,000 pounds per QALY threshold.

United Kingdom's National Health Service Drug Spending Priorities Questioned in New Analysis

At the HIV Analyst &amp; Investor Event on Tuesday, Gilead Sciences Inc. (NASDAQ:<a class="ticker" href="https://www.benzinga.com/stock/GILD#NASDAQ">GILD</a>) revealed that it is developing a once-yearly injectable formulation for lenacapavir.
The annual formulation is currently in Phase 1 study, with data to be presented.
In its investor presentation, the company said it <a target="_blank" href="https://s29.q4cdn.com/585078350/files/doc_events/2024/Dec/10/GILD-HIV-Analyst-Investor-Event-Presentation-10-December-2024.pdf" rel="noreferrer noopener">expects</a> the pivotal Phase 3 pharmacokinetics study&#8217;s first patient to be in 2025, and potential regulatory filings will begin in late 2027.
In September, Gilead revealed the results of an interim analysis from a second Phase 3 trial investigating the use of the company&#8217;s twice-yearly injectable HIV-1&nbsp;<a target="_blank" href="https://www.benzinga.com/general/biotech/24/09/40840139/gilead-unveils-remarkable-outcomes-from-late-stage-lenacapavir-study-showing-promise-in-preventin" rel="noreferrer noopener">capsid inhibitor, lenacapavir</a>.
Lenacapavir reduced HIV infections by 96% compared to background HIV incidence.

In November, the&nbsp;New England Journal of Medicine&nbsp;published the full results from Gilead&#8217;s Phase 3 PURPOSE 2 trial evaluating twice-yearly lenacapavir for HIV prevention.
The study found that twice-yearly lenacapavir for pre-exposure prophylaxis (PrEP) was highly efficacious at reducing HIV infections, reducing them by 96% compared to <a target="_blank" href="https://www.benzinga.com/pressreleases/24/11/b42225869/gilead-announces-new-england-journal-of-medicine-publication-of-purpose-2-results" rel="noreferrer noopener">background HIV incidence</a>.
Twice-yearly lenacapavir for PrEP also demonstrated superiority to once-daily Truvada&nbsp;(emtricitabine 200 mg and tenofovir disoproxil fumarate 300 mg; F/TDF) for PrEP and was generally well-tolerated, with no significant or new safety concerns identified.
In 2021, the FDA approved ViiV Healthcare&#8217;s Apretude, the first injectable form of PrEP administered every two months.
Pfizer Inc (NYSE:<a class="ticker" href="https://www.benzinga.com/stock/PFE#NYSE">PFE</a>) and GSK Plc (NYSE:<a class="ticker" href="https://www.benzinga.com/stock/GSK#NYSE">GSK</a>) own ViiV Healthcare.
STAT News <a target="_blank" href="https://www.statnews.com/2024/12/11/hiv-prevention-gilead-once-a-year-prep/?utm_campaign=rss" rel="noreferrer noopener">highlights</a> that Gilead plans to seek approval for a PrEP injection that requires only two clinic visits per year. This less frequent dosing has been shown to be more effective than traditional daily oral PrEP.
However, global access to these new PrEP options remains a challenge. Former CDC director Rochelle Walensky criticized ViiV for not prioritizing accessibility in Africa and urged Gilead to ensure broader availability <a target="_blank" href="https://www.benzinga.com/general/biotech/24/07/39940874/gileads-42-5k-priced-hiv-drug-could-be-made-at-just-40-annually-campaigners-urge-company-to-slash" rel="noreferrer noopener">of its drug lenacapavir</a>.
Unlike vaccines, which train the immune system to recognize and fight viruses long-term, PrEP relies on antivirals that are effective only while active in the body. The main challenge has been developing drugs that remain effective for extended periods.
While significant progress has been made with long-lasting PrEP, the same cannot be said for HIV vaccines.
Efforts to create a vaccine have struggled against the virus&#8217;s complexity, and no vaccines are currently in late-stage trials following recent setbacks.
The success of long-term PrEP may further complicate vaccine development by reducing the urgency for alternatives.

Gilead Unveils New Yearly HIV Injection To Offer Hope for Less Frequent Preventive Dosing

On Monday, GSK plc (NYSE:<a class="ticker" href="https://www.benzinga.com/stock/GSK#NYSE">GSK</a>) announced statistically significant and clinically meaningful overall survival (OS) results from a planned interim analysis of the <a target="_blank" href="https://www.benzinga.com/pressreleases/24/12/b42397184/belantamab-mafodotin-shows-significant-overall-survival-benefit-reducing-the-risk-of-death-by-42-i" rel="noreferrer noopener">DREAMM-7 trial</a>.
The trial assessed Blenrep (belantamab mafodotin) in combination with bortezomib plus dexamethasone (BVd) versus Johnson &amp; Johnson&#8217;s (NYSE:<a class="ticker" href="https://www.benzinga.com/stock/JNJ#NYSE">JNJ</a>) Darzalex (daratumumab) in combination with bortezomib plus dexamethasone (DVd) as a second line or later treatment for relapsed or <a target="_blank" href="https://www.benzinga.com/general/biotech/24/11/41986367/gsk-raises-prospects-for-withdrawn-blood-cancer-drug-blenrep-to-return-to-us-market-heres-why" rel="noreferrer noopener">refractory multiple myeloma</a>.
These data were featured at the American Society of Hematology Annual Meeting and Exposition.
With a median follow-up of 39.4 months, the analysis presented shows a statistically significant 42% reduction in the risk of death among patients receiving the Blenrep combination (n=243) versus the Darzalex arm (n=251) (HR 0.58).

Although the median overall survival (mOS) was not reached in either arm of the study, the projected mOS for BVd is 84 months compared to 51 months for the Darzalex combo arm.
The three-year overall survival rate was 74% in the Blenrep combination arm and 60% in the daratumumab combination arm.
The survival benefit favoring Blenrep over Darzalex was seen as early as four months and was sustained over time.
The Blenrep combination also showed statistically significant superiority over the Darzalex combination on the key secondary endpoint of minimal residual disease (MRD) negativity (no detectable cancer cells).
The greater than 2.5-fold improvement in the rate of MRD negativity seen at the time of the primary analysis for patients who received BVd can now be declared statistically significant after the positive overall survival readout based on the predefined testing procedure.
In addition to OS and MRD negativity, the Blenrep combination resulted in clinically meaningful improvements in all key secondary efficacy endpoints compared to the daratumumab combination, including duration of response (DOR) and progression-free survival.
Eye-related side effects, a known risk of treatment with Blenrep, were generally manageable and resolvable with dose modification and led to a low (10%) treatment discontinuation rate.

On Oct. 23, Vertiv reported third-quarter results. Net sales rose 19% year-over-year to $2.074 billion, beating the consensus of $1.977 billion. Organic orders (excluding foreign exchange) rose 17%. Adjusted operating profit surged 41% to $417 million, with the margin expanding 310 basis points to 20.1%, driven by higher volumes and favorable commercial execution.
