To help drive down patient readmissions and enhance care, Northwell Health will integrate predictive artificial intelligence (AI) software from Jvion's Cognitive Machine into the health system's electronic medical records (EMR) at 15 hospitals.

To help drive down patient readmissions and enhance care, Northwell Health will integrate predictive artificial intelligence (AI) software from Jvion's Cognitive Machine into the health system's electronic medical records (EMR) at 15 hospitals.
Georgia-based Jvion and its AI solution will be used to identify at-risk patients and other metrics that contribute to an individual's increased risk. This includes social determinants such as a lack of access to transportation as well as food and housing insecurity. During the initial phase of implementation, Northwell Health will target readmissions and avoidable admissions within its managed care population.

The goal is to seamlessly integrate the Cognitive Machine's risk and recommendation outputs so that caregivers have access to the information within the existing clinical workflow. Jvion's solution will proactively manage patients by flagging those at-risk while providing the clinical drivers and other factors that contribute to the likelihood a readmission or avoidable admission. It will also deliver individualized recommended actions to best mitigate risk on an individual basis.
